We the undersigned as players, legal parents or guardians, verify that the players listed are in good health and able to fully participate in this event. In addition, the birth date listed is correct. We recognize that all sports have some risk of injury.

We hereby release San Diego Sol, Inc. and all host sites and all officers, directors, owners and staff affiliated with these organizations (collectively the Indemnities), from any and all liability claims for injury, illness and loss sustained by the participants while playing, practicing and traveling to and from that may be incurred during this event.

The undersigned (as participants and guardians) assume all risks and absolve, indemnify and hold the Indemnities harmless of any and all liability or damage, injury, or expense of any kind arising out of, or connected with this event.
